The nurse will apply Relationship Based Care, set daily goals, and coordinate care with the care team to ensure safe, holistic patient experiences. A strong focus on communication, learning, and continuous improvement is expected.

Responsibilities

• Demonstrates knowledge of adult learning principles and teaching patients, families, students and new staff.
• Provides detailed and appropriate teaching to patients and families to guide episodes of care and transitions to home or higher level of care.
• Supports development of students, new staff and colleagues and may serve as a preceptor.
• Creates an environment of open dialogue and continuous development by seeking feedback and improving practice.
• Demonstrates accountability for professional development and quality of care; participates in unit-based governance and supports change processes.
• Maintains productive relationships, respects cultural differences, and communicates safety hazards.
