Responsibilities

• Perform evaluation and re-evaluation of patients.
• Gather pertinent data and determine treatment diagnosis.
• Develop and implement an individualized plan of care.
• Coordinate with interdisciplinary team and communicate progress.
• Establish discharge plan and patient caregiver education.
