Job summary

The Keystone Distinction Group is seeking a dedicated Registered Nurse (RN) to join our Casper, WY home health team. This full-time role focuses on delivering compassionate in-home nursing care to seniors, with per diem shifts and flexible hours.

You will assess patients, develop care plans, administer treatments, and educate families while coordinating with an interdisciplinary team in the comfort of patients' homes.

Qualifications

• Active Wyoming RN license required.
• Minimum 0 years of experience in home health or related setting.
• Flexibility to work evenings, weekends, and holidays.

Responsibilities

• Provide skilled nursing care to seniors in their homes, assessing physical, emotional, and social needs.
• Develop and implement individualized care plans with patients, families and team.
• Administer medications, treatments, and therapies as prescribed, monitor responses.
• Educate patients and families on disease management, self-care, and wellness.
• Collaborate with healthcare professionals to ensure coordinated care.
• Maintain accurate records of patient interactions, assessments and care plans.
