Job summary

Mexican American Opportunity Foundation is seeking a Case Worker for the Senior Services Program in Montebello, on-site. You will assess elderly clients, develop individualized care plans, and coordinate services to promote independence and prevent unnecessary institutionalization.

The role requires a high school diploma (Bachelor's preferred), 3+ years of experience in elderly social services, and bilingual English/Spanish proficiency.

HOURLY SALARY: $23.54 - $25.42
FLSA STATUS: Non-Exempt, Full-Time

GENERAL SUMMARY

Under the direct supervision of the Program Director, the Case Worker is responsible for assessing the needs of elderly clients and developing and implementing appropriate service plans to support independence and help prevent unnecessary institutionalization. This is an on-site position only.

DUTIES AND RESPONSIBILITIES

The Senior Program Case Worker's duties include, but are not limited to, the following:

  • Work in both office and field settings and is required to drive and travel to meet with clients, attend meetings, and conduct community outreach.
  • Conduct client interviews to identify needs, concerns, and appropriate services.
  • Complete comprehensive assessments and develop individualized service and care plans.
  • Manage an assigned caseload, including assessments, service authorization and coordination, case monitoring, follow-up, and community outreach.
  • Provide telephone reassurance and support to clients who are isolated, live alone, or require additional assistance.
  • Consult and coordinate with staff regarding client referrals and available services.
  • Maintain accurate, complete, and confidential records of client contacts, assessments, referrals, and services provided.
  • Assist clients with translation, completion of forms, and communication with external service providers.
  • Advocate on behalf of clients to help ensure access to appropriate programs, benefits, and community resources.
  • Support the implementation of Senior Services Program components, including nutrition services, information and assistance, and evidence-based programs.
  • Follow applicable state-mandated standards and program requirements for case management services.
  • Meet with clients in office, home, and community settings and attend meetings and outreach activities as required.
  • Perform other related duties as assigned by the Program Director.
MINIMUM REQUIREMENTS
  • High school diploma or equivalent required.
  • Bachelor's degree in Social Sciences, Gerontology, or a related field preferred.
  • Minimum of three (3) years of work experience providing social services to elderly populations.
  • Bilingual English/Spanish proficiency required.
  • Must possess a valid driver's license, reliable transportation, and current automobile insurance.
  • Proficiency in Microsoft Word and general computer applications.
  • Ability to work evenings or weekends when required by program needs.
  • Demonstrated ability to work effectively with clients on a one-on-one basis and establish positive working relationships.
  • Strong organizational, communication, documentation, and time-management skills.
  • Ability to work independently while also contributing effectively in a team environment.
  • Ability to work effectively with diverse populations and demonstrate cultural sensitivity.
  • Ability to maintain confidentiality and protect sensitive client and program information.
  • Ability to represent MAOF professionally and maintain a working knowledge of program services and community resources.
PHYSICAL DEMANDS

The physical demands described below are representative of those required to perform the essential functions of the position.

While performing the duties of this position, the employee is regularly required to use hands and fingers to handle materials, operate office equipment, and perform computer and documentation tasks. The employee is frequently required to sit, reach, communicate verbally and in writing, and use a telephone or other communication equipment.

The employee is occasionally required to stand, walk, and move throughout office, client, and community settings. This position requires regular driving and travel to client and community locations.

The employee may occasionally be required to lift or move items weighing up to twenty (20) pounds. Required vision abilities include close vision, peripheral vision, and the ability to adjust focus.

WORK ENVIRONMENT

This is an on-site position only. The Senior Program Case Worker works in both office and field settings and is required to travel to client homes, community locations, meetings, and outreach activities.

The position involves frequent interaction with elderly clients, family members, coworkers, community partners, service providers, and members of the public. While working in the field, the employee may encounter varying environmental conditions and is expected to maintain professionalism, confidentiality, and appropriate safety practices at all times.
