Summary:
Under the supervision of the Program Manager and/or Team Leader, provides case management, rehabilitation, crisis interventions, and outreach services in home, office, and community settings to adults with serious long-term mental illness. Collaborates with psychiatric and allied mental health teams in assessment, diagnosis, intervention, and treatment planning. Conducts substance abuse treatment in accordance with the 12 core functions of substance abuse counseling as defined by the ICRC.
This position will have duties and responsibilities in both the Pawtucket (103 Bacon St.) and Johnston (1443 Hartford Avenue) offices.
Responsibilities:
- Provides clinical interventions such as resource linking, resource modification, skill teaching, and programming to consumers on Mobile Treatment Team caseload, including but not limited to:
- Assisting consumers in obtaining financial benefits, medical and dental care, employment, and affordable housing.
- Developing social support networks and participating in social and leisure activities.
- Applying ADL skills in real-life environments.
Conducts comprehensive clinical assessments and develops interventions for substance abuse, sexual abuse, symptom management, and family support and education.
Assesses suicidal and homicidal behavior.
Coordinates emergency psychiatric services, documenting interventions, options considered, and client responses.
Provides coordination and monitoring of treatment services, including:
- 22 hours of direct client services per week.
- Documentation in progress notes as per procedures.
- Care coordination with other staff and agencies.
Participates in treatment planning, assessment, and documentation processes.
Provides psychoeducational services to clients and families, including information on mental illness, substance abuse, and parenting.
Arrives on time for scheduled services, modifies treatment plans as needed, and documents all activities.
Assists with discharge planning and maintains accurate medical records adhering to policies and confidentiality standards.
Attends community and agency meetings punctually and participates actively.
Facilitates inter-agency collaborations and participates in staff meetings and treatment reviews.
Provides ongoing assessment of client symptoms and response to treatment.
Supports clients in finding and maintaining housing, managing finances, accessing transportation, and maintaining healthcare.
Plans and prompts social and leisure activities outside of regular hours.
Prepares for supervision and monitors medication adherence and side effects.
Performs other duties as assigned.

Qualifications:
- Associate’s degree required; Bachelor’s preferred.
- Two years of experience in Human Services preferred.
- Experience with substance abuse, sexual abuse, or criminal justice involvement is a plus.
Necessary Special Requirements:
- Valid driver’s license and reliable transportation during work hours.
- Automobile liability insurance as per state regulations.
