Job summary

The Salvation Army in Sherman, TX is seeking a dedicated case manager to support clients in an emergency day shelter program. You will administer long-term case management, create tailored plans, and coordinate services.

You will mentor and counsel clients, advocate for access to needed services, maintain accurate records, and ensure funding and program compliance. The role requires strong communication, organizational skills, and a valid driver’s license.

Qualifications

  • Associate degree or two years of college in Social Work or related field.
  • Two years of experience in social/public service with case management.
  • Ability to read, write, and communicate effectively in English.
  • Valid State Driver's License.

Responsibilities

  • Monitors assigned caseload in an emergency day shelter program.
  • Creates case plans based on the client’s history to determine effective programs.
  • Develops goals and evaluates progress through mentoring and counseling sessions.
  • Advocates for clients to obtain services to cope with their environment.
  • Refers clients to Salvation Army programs or other agencies as needed.
  • Assists in onsite food pantry and maintains case records.

Schedule/Hours: M – F 8:00 a.m. – 5:00 p.m.Monitors assigned caseload of clients participating in an emergency day shelter program; establishes program plan/goals and evaluates client's progress by conducting routine meetings with client and/or staff; serves as advocate for client in order to acquire services that will enable them to functionally cope with their environment.Key Responsibilities:Interviews, accepts, and provides comprehensive, long-term, structured, complex, case management services for an assigned caseload of clients participating in an established life management programCreates case plan based upon the uniqueness of the client’s history in order to determine most effective program plans for the individual's success.Develops goals and evaluates client's progress by conducting mentoring and counseling sessions with client and/or staff;Serves as advocate for client in order to acquire services that will enable them to functionally cope with their environment; ensures constant compliance with funding requirements.Refers clients to appropriate Salvation Army programs or other agencies based on clients’ needs and in accordance with their program plan.Assists in onsite food pantry.Prepares and maintains case records and logs on all assigned clients; ensures the accuracy and completeness of the same.Physical Requirements and Working Conditions:Ability to meet attendance requirements.Ability to read, write, and communicate the English language effectively.Ability to communicate clearly and effectively both orally and in writing.Ability to think clearly and quickly in order to maintain control of client caseload.Ability to effectively and efficiently work on multiple cases at the same time without becoming frustrated or disorganized.Limited amount of physical effort required associated with walking, standing, lifting and carrying light objects (less than 25 lbs.) 5-10% of work time.Duties are usually performed seated.
