Baylor College of Medicine in Houston is seeking a Case Worker to help clients access resources, develop care plans, and coordinate a continuum of care across mental health, medical services, and basic needs.
The role involves collaborating with clinicians and community partners, conducting needs assessments, creating negotiated care plans, and maintaining detailed client records to document outcomes and program effectiveness. This position is located at our McNair Campus.
The Case Worker will provide help in accessing available resources, development of care plans, information referral, and advocacy for clients. Works with clinicians and other staff to provide clients access to a continuum of care addressing health care needs and other critical resources, such as therapy and psychiatry services, basic needs, entitlement benefits, and other long term care needs.
