Job summary

Cardinal McCloskey Community Services (CMCS) seeks a Case Planner in New York City to assess families and coordinate services. You will develop individualized plans, collaborate with providers, and support up to 13 families while meeting ACS caseload standards.

Requires a BSW or bachelor’s in human services and knowledge of the child welfare system. English and Spanish bilingual abilities are a plus; travel across the city is expected. CMCS offers comprehensive benefits and flexible schedules.

Responsibilities

• Conduct comprehensive assessments and develop individualized service plans.
• Coordinate multidisciplinary interventions tailored to each family's needs.
• Meet ACS minimum casework contact standards for a caseload of up to 13 families.
• Build meaningful relationships with families over an average case duration of 12 months.
• Collaborate with internal and external providers to ensure safety, stability, and well-being.

Case Planner will work directly with children and families facing complex challenges, including serious medical conditions, substance use disorders, mental health concerns, and other circumstances that may impact family stability. Through assessment, case management, advocacy, and resource coordination, the Case Planner will support families in achieving their goals and maintaining safe, nurturing home environments.

Responsibilities:

• Makes diligent efforts to meet the ACS minimum casework contact standards for a caseload of up to 13 families.

• BSW or bachelor's degree in human services.
• Some knowledge of the child welfare system.

• Fluency in English and Spanish a Plus
• Citywide travel required
