Job summary

Cardinal McCloskey Community Services in New York City is seeking a Case Planner who will work directly with children and families facing complex challenges, providing assessment, case management, advocacy, and resource coordination to promote safe, nurturing homes.

The role requires a BS/BSW in human services, knowledge of the child welfare system, and willingness to travel citywide. Bilingual English/Spanish is a plus; benefits include medical, dental, 403(b), and generous PTO.

Qualifications

• BSW or bachelor's degree in human services.
• Some knowledge of the child welfare system.
• Fluency in English and Spanish a plus.

Responsibilities

• Conduct comprehensive assessments and develop individualized service plans.
• Coordinate multidisciplinary interventions tailored to each family's unique needs.
• Makes diligent efforts to meet the ACS minimum casework contact standards for a caseload of up to 13 families.
• Build meaningful relationships with families over an average case duration of 12 months.
• Collaborate with internal and external providers to ensure safety, stability, and well-being.

Case Planner will work directly with children and families facing complex challenges, including serious medical conditions, substance use disorders, mental health concerns, and other circumstances that may impact family stability. Through assessment, case management, advocacy, and resource coordination, the Case Planner will support families in achieving their goals and maintaining safe, nurturing home environments.
