Job summary

Cardinal McCloskey Community Services (CMCS) is seeking a Case Planner in New York to work directly with children and families facing challenges. You will conduct assessments, plan services, advocate for families, and coordinate resources to promote stability and safety.

The role involves building strong relationships, traveling citywide, and coordinating with multiple providers to improve outcomes for up to 13 families over an average 12-month caseload.

Qualifications

• BSW or bachelor's degree in human services required.
• Some knowledge of the child welfare system expected.
• Fluency in English and Spanish is a plus.
• Citywide travel required.

Responsibilities

• Conduct comprehensive assessments and develop individualized service plans.
• Coordinate multidisciplinary interventions tailored to each family's needs.
• Meet ACS minimum casework contact standards for caseloads.
• Build relationships with families over an average case duration of 12 months.
• Collaborate with internal and external providers to ensure safety and well-being.

Through assessment, case management, advocacy, and resource coordination, the Case Planner will support families in achieving their goals and maintaining safe, nurturing home environments.

• Makes diligent efforts to meet the ACS minimum casework contact standards for a caseload of up to 13 families.
• Build meaningful relationships with families over an average case duration of 12 months.
• Collaborate with internal and external providers to ensure safety, stability, and well-being.

Requirements:

• BSW or bachelor's degree in human services.
• Some knowledge of the child welfare system.
