Job summary

Southern Highlands Community Mental Health Center in Wyoming County, WV is seeking a full-time Case Manager/Care Coordinator Level 2 for School-Based Services. You will work with children, adolescents and families to connect them with behavioral, medical, social, educational and health-related supports.

The role coordinates care with families, school personnel and providers to advocate for students and implement person- and family-centered treatment plans.

Qualifications

• Education requirements include a bachelor’s or master’s degree in a human-services field per WV Social Work board.
• Alternatively, a licensed psychologist with a master’s or doctoral degree.
• Experience with school-based services is preferred.

Responsibilities

• Assist children, adolescents, and families in accessing needed behavioral, medical, social, educational, and health-related services and supports.

Southern Highlands Community Mental Health Center is seeking a full-time Case Manager/Care Coordinator Level 2 to join our School-Based Services team in Wyoming County.

This position works with children, adolescents, and their families to help connect them with essential behavioral, medical, social, educational, and health-related supports. The Case Manager/Care Coordinator works closely with families, school personnel, and service providers to coordinate care, advocate for students, and support the development and implementation of person- and family-centered treatment plans.

Key Responsibilities:
