Job summary

The Salvation Army Owens Campus is seeking a Full-Time Case Manager for the CIP program to deliver comprehensive veteran services. You will coordinate care, manage a caseload of up to 21 veterans, and collaborate with VA personnel to ensure access to health and supportive services.

Responsibilities include intake/discharge, data entry, life skills sessions, and outreach to shelters and treatment programs. Strong communication, case management, and driving capability are essential.

Qualifications

  • Bachelor’s degree in a Human Service related discipline; substitution with 3 years experience allowed
  • Experience working with homeless and/or at-risk veterans preferred
  • Experience in case management and/or individual counseling
  • Strong oral and written communication skills
  • Proficiency in Microsoft Office (Word, Excel, etc.)
  • Ability to work effectively with residents, staff and community partners
  • Nevada Driver License with clean record; background check required
  • Veteran Status preferred

Responsibilities

  • Track progression of veterans through the program.
  • Oversee medication management and facility compliance.
  • Assist veterans with employment readiness.
  • Conduct outreach to shelters and related facilities.
  • Maintain participant confidentiality at all times.
  • Attend team meetings and trainings.
  • Perform other duties and special projects.

Qualifications
  • Bachelor’s degree in a Human Service related discipline. Minimum three years of related employment experience may be substituted for degree requirement.
  • Experience working with homeless and/or at-risk veterans preferred.
  • Experience in case management and/or individual counseling.
  • Strong oral and written communication skills.
  • Proficiency in Microsoft Office Software (Word, Excel, etc.)
  • Ability to communicate and work effectively with a diverse group of residents, staff and community partners.
  • Must have a valid Nevada Driver License and clean driving record.
  • Must be able to successfully pass a criminal background check.
  • Veteran Status Preferred.

The Full-Time Case Manager for the CIP program reports directly to Veteran Services Coordinator and is responsible for the coordination and comprehensive services delivered to veterans by The Salvation Army Veteran Services.

Responsibilities
  • Provide direct case management services to all veteran residents with focus on CIP participants.
  • Manage a caseload of up to 21 veteran residents.
  • Facilitate the intake and discharge process as well as client orientation.
  • Complete accurate and timely data entry into the HMIS system.
  • Coordinate care with VA personnel and others on behalf of participants for eligibility documentation, mental and physical health services.
  • Facilitate life skills, informational, and educational classroom presentations and group sessions.
  • Facilitate weekly one on one sessions with veteran residents.
  • Transport veteran residents to appointments, if appropriate.
  • Maintain client databases and referrals for services and generate reports.
  • Maintain, record, and report progression of veterans through program participation.
  • Oversee medication management.
  • Oversee compliance with all facility rules.
  • Assist veterans to identify mainstream needs and facilitate the process with appropriate service providers to meet those needs.
  • Assist veterans with employment readiness services.
  • Conduct outreach activities, including visits to facilities such as shelters, treatment programs, soup kitchens and other locations where homeless veterans may congregate.
  • Maintain participant confidentiality at all time.
  • Perform interventions and crisis-oriented services.
  • Attend all scheduled team meetings and trainings.
  • Other duties and special projects as assigned.
