Responsibilities

• Complete biopsychosocial assessments for each member.
• Conduct bi-monthly home visits and maintain member records.
• Coordinate care with other treatment providers and evaluate Direct Care Aide performance.
• Participate in case conferencing
• Provide direct member care as needed
• Coordinate care with other treatment providers
• Evaluation of direct participant care performed by the Direct Care Aide
• Develop, along with the nurse, the member plan of care to be carried out by the Direct Care Aide (DCA)
• Collect and document members current health providers and make additional referrals as necessary
• Work as part of a multidisciplinary team alongside DCA, Nurse, and Program Coordinator along with other treatment providers the members may see
• Respond to telephone inquiries relating to members requests and/or referrals for additional services.
• Follow ups on doctor's appointments, medical releases, and medical equipment

Part‑time Care Manager

Care Manager Responsibilities

• Completion of biopsychosocial for each member
