Job summary

Meridian Senior Living is seeking a Case Manager to support residents’ social, mental, spiritual and emotional well-being. You will oversee care plans, supervise staff, and coordinate with the Wellness and Executive Directors to ensure effective service delivery.

Qualified candidates have a Bachelor's/Master's in Human Services or related field, or an Associate with geriatric experience, and an LPN is required.

Qualifications

• Bachelor’s or Masters in Human Services, Case Management, Social Work or equivalent with 1 year full-time experience with adults/geriatric population.
• Associate Degree with 3 years of geriatric experience.
• LPN required.
• Ability to work flexible hours and weekends.
• Strong organizational and time management skills.
• Ability to respond calmly to emergencies.
• Ability to communicate effectively with residents, families, staff, vendors and the public.
• Must promote Meridian Policies and privacy and maintain health requirements.

Responsibilities

• Oversee all record keeping practices for residents and ensure efficiency.
• Assist in the wellness department and coordinate care plans with the Wellness Director and Executive Director.
• Supervise Care Staff and Nursing staff; assist with training and inservices.
• Coordinate family meetings, staffing/scheduling and on-call every other week.
• Assist with Med Cart as needed.

Case Manager Position

Qualified applicants will need to be approved by NYS DOH

Help residents achieve and maintain optimum social, mental, spiritual and emotional well-being. Serve as a resource for residents and their families. Oversee all record keeping practices and ensure they are effective and efficient. Assist in the wellness department and work closely with Wellness Director and Executive Director. Supervise Care Staff and Nursing staff. Assist with training, inservices and coordination of care plans, family meeting, staffing/scheduling and On-call every other week. Assist on Med Cart as Needed.

Must have LPN

Sunday-Thursday position 9am-5:30pm.

Qualifications

• Collegiate degree and related experience, specifically:
• A Bachelor’s or Masters’ degree from an accredited college or university in Human Services, Case Management, Social Work or the equivalent, and one year of full-time experience working with the adult or geriatric population; OR
• An Associate Degree from an accredited college or university in Human Services, Case Management, Social Work, or the equivalent, and three years of full-time experience working with the adult or geriatric population.
• LPN required
• Ability to work flexible hours and weekends.
• Strong organizational and time management skills.
• Ability to respond calmly to emergencies.
• Possesses good judgment, problem-solving and decision-making skills.
• Successfully complete training as required by Meridian in accordance with State regulations.
• Ability to communicate effectively with residents, families, staff, vendors and general public.
• Must have compassion for and desire to work with the elderly.
• Must demonstrate the ability to work responsibly as a team member as well as an individual.
• Must be honest, fair, dependable, respect confidentiality and the rights and privacy of others and local, state and federal laws.
• Must practice and promote Meridian Policies and Procedures, Mission Statement, Core Values and Founding Principles.
• Must meet all health requirements.
• Must pass criminal background check.
• Must be able to perform duties and responsibilities (essential job functions), with or without reasonable accommodation.
