PATH is seeking passionate candidates to join the Site Based Housing team as a Case Manager at our San Jose office. This role focuses on helping clients break the cycle of homelessness through individualized case management, coordinating services, and supporting clients in achieving housing and health outcomes.

Responsibilities
  • Conduct intake and individualized needs assessment for all participants and collaborate to develop Individualized Service Plan (ISP) that addresses housing goals, income, and other personal goals including medical, mental health, substance use, financial resources, vocational, and social support needs.
  • Provide referrals, linkages, information, and support to resources that help participants achieve their ISP goals.
  • Complete program‑specific assessments (e.g., VI‑SPDAT).
  • Evaluate strengths and challenges to address short‑term and long‑term goals, conduct a 90‑day review with the participant after initial intake.
  • Maintain documentation standards per PATH policies, completing case file notes promptly and ensuring a complete, accurate, and current participant file with all required documents.
  • Maintain confidentiality of participant files per HIPAA and all applicable guidelines.
  • Develop therapeutic or professional relationships with clients and systems/partners that enhance engagement and retention in housing.
  • Provide crisis intervention and risk assessment services focused on safety and client empowerment.
  • Use motivational interviewing and trauma‑informed care in service delivery.
  • Maintain current, thorough knowledge of community resources and utilize them to provide comprehensive, wrap‑around services to clients.

Preferred Qualifications
  • Two (2) years of experience in healthcare, case management, homeless services, or working with vulnerable populations or a related field.
Minimum Qualifications
  • All levels of education and experience are welcome.
Mandatory Requirements
  • Employment eligibility verification.
  • CPR/First Aid training (or willingness to obtain).
  • Successful completion of Tuberculosis Test, Background Screening, and Drug Test as a condition of hire.
Driving
  • Reliable transportation.
  • A valid driver’s license.
  • Proof of insurance and ownership for personal vehicles used during work duties.
  • Ability to qualify for PATH’s insurance coverage.
Employment Eligibility

Applicants must be authorized to work for any employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa. PATH will not employ individuals currently receiving services or those who have received services within the past two years.

California‑Based Work Expectations

All work must be performed within California. Approval for work outside the office does not allow work outside California. PATH does not offer fully remote positions. Some roles may include hybrid schedules involving multiple work sites or approved off‑site locations; however, work will not be performed from home.
