Since its foundation in 1984, PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting our clients to a comprehensive continuum of homelessness prevention, street outreach, employment preparation and placement assistance, individualized case management, supportive services, interim housing, and permanent supportive housing.
As part of the Santa Clara team, the Case Manager role’s main function is to assist clients in breaking the cycle of homelessness by utilizing a “whatever it takes” approach. The position supports clients through all phases of homelessness—including on the street, interim housing, transitional housing, and permanent housing—and helps them access and maintain necessary services in healthcare, social services, interpersonal support systems, and a coordinated entry system (CES). The role provides individualized supportive services by developing a plan that addresses strengths, barriers, health needs, income, and stable housing. The professional will identify areas of assistance to achieve the outlined goals and focus on housing and health as necessary outcomes for success.
