Job summary

Walden Family Services is seeking a Case Manager for the Transitional Housing Program in Riverside, CA. You will work with up to 12 non minor dependents, coordinating with placing agencies and adjunct treatment providers to help clients reach their goals.

Responsibilities include assessing needs, developing service plans, documenting progress, and coordinating referrals. A bachelor’s degree in social work or related field, California driver’s license, and DOJ/Fingerprint clearances are required.

Qualifications

• Bachelor’s degree in social work or closely related field required.
• Minimum 1 year experience with the population (NMD).
• Valid California Driver’s License with good driving record.
• Fingerprint clearance and DOJ background checks.
• Must meet Community Care Licensing requirements for licensed facilities.

Responsibilities

• Be knowledgeable of transitional housing programs and resources for NMD.
• Conduct intake and needs/services planning; maintain accurate records.
• Provide weekly consultation and support; assist in budgeting and housing advocacy.
• Coordinate referrals to medical, therapy, parenting classes, job training, etc.
