Job Description

Job Title: CASE MANAGER SUPERVISOR / SENIOR CASE MANAGER

Division: Services

FLSA: Non-Exempt

Reports To: Shelter Director

Location: Queens, NY

Salary: $45,000 - $50,000

Summary

The Case Manager Supervisor / Senior Case Manager will supervise the case management team within a homeless shelter. Collaboration with other on-site departments is essential to facilitate effective problem-solving and strategy alignment. Proof of COVID-19 vaccination is required.

Responsibilities

• Manage intake and develop comprehensive assessments for new participants.
• Create systems to ensure quality and efficiency of casework services.
• Ensure staff appropriately identifies client needs, provides referrals, and follows up to promote successful achievement of service plan goals.
• Document assessments, service plans, and client interactions properly.
• Review case management documentation and data collection tools.
• Supervise service staff.
• Maintain professional interventions consistent with agency policies and evidence-based practices.
• Build collaboration with community service providers.
• Maintain and submit required documentation and reports promptly.
• Provide crisis intervention and stabilization as needed.
• Identify program needs and make recommendations.
• Perform related tasks as assigned.

Qualifications

• At least 3 years of relevant experience with supervisory responsibilities.
• Fluency in Spanish is highly desirable.
• Ability to work some evenings and weekends.
• Excellent writing and assessment skills.
• Experience in counseling, community outreach, group leadership, and program development preferred.
• Proficiency in MS Word, Excel, and database management.
• Own car and valid NYS driver’s license required.
