Job summary

FHE Health in Boca Raton, FL is seeking a Case Manager to support patients through treatment, from orientation through discharge planning. You will serve as a liaison between patients, the clinical team, and admissions staff.

Responsibilities include conducting comprehensive assessments, coordinating access to therapy and medical services, monitoring progress, and providing ongoing support and counseling. A bachelor's degree in a human services field and CPR certification are preferred.

Qualifications

• Knowledge of addiction and recovery required or preferred.
• Familiarity with CARF, DCF and Joint Commission standards preferred.
• Strong interpersonal and communication skills for working with patients, families and interdisciplinary teams.
• Knowledge of mental health conditions, treatment modalities and community resources.
• Bachelor’s degree in social work, psychology, counseling or related human services field preferred.
• Prior experience in behavioral health, mental health, or substance use treatment setting preferred.
• Current CPR Certification.
• Crisis Prevention Intervention (CPI) Training.
• Successful clearance of a Level II background check.
