Job summary

Akua Mind Body in Long Beach is seeking a full-time Case Manager to join their team in the Substance Use Prevention Treatment Program. The role combines clinical expertise and administrative duties focusing on client-centered care.

The successful candidate will conduct ASAM assessments, manage billing in the Smart Care EHR platform, and facilitate group therapy sessions. The position offers health insurance and equity shares.

Qualifications

• Must possess CADC or SUDCC Certification or higher.
• At least 1 year of experience in substance abuse treatment or behavioral healthcare.
• Valid CA Driver's License and current health clearance required.

Responsibilities

• Conduct comprehensive ASAM assessments for clients.
• Develop personalized treatment plans to address recovery goals.
• Facilitate psychoeducational groups on recovery and life skills.
• Connect clients to various resources and services.
• Maintain regular contact for ongoing support and crisis intervention.
• Collaborate with treatment teams for coordinated care.
• Maintain accurate client documentation.
• Manage crises and coordinate emergency services when necessary.
