Qualifications

• Bachelor's or Master's Social Work degree required/preferred depending on role.
• Licensed Baccalaureate Social Worker (LBSW) or Licensed Master Social Worker (LMSW) required in South Carolina.

Responsibilities

• Coordinate resources and services for individual patient needs and fiscal realities.
• Complete Patient Choice Form and track clinical service utilization from admission to discharge.
• Analyze data for appropriateness and timing of interventions and communicate findings to nurses and managers.
• Support timely delivery of services, reduce length of stay, and improve patient outcomes while managing costs.
