Job summary

Commonwealth Charter Academy is seeking a Case Manager - Social Work to provide social-emotional and behavioral support for students from a Family Service Center in Harrisburg, PA. You will collaborate with teachers, administrators, and caretakers to develop interventions and service plans, and you will act as a trusted liaison for students and families.

Ideal candidates have a Bachelor’s degree in a related field, with 2+ years of relevant experience; Master’s and PA LSW/LCSW are preferred.

Qualifications

Responsibilities

  • Serve as a resource to administrators, counselors, teachers, and health staff regarding students and their social-emotional well-being.
  • Perform intake assessments; implement case management services; collaborate to develop interventions and service plans.
  • Interpret needs and provide guidance on attendance, family matters, academics, policies, and record-keeping.
  • Work with students and families to address disabilities, trauma, and behavior concerns.
  • Lead the Student Assistance Program team and assist with cases involving mental health and substance-use concerns.
  • Monitor progress and conduct home visits to assess family environment when needed.
  • Assist with attendance, truancy mitigation, and engagement supports.
  • Serve as Homeless Liaison to connect families with shelters and social services.
  • Support multi-tiered systems of support and behavior interventions with counselors.
  • Provide mentoring and connect families to internal or external supports; communicate needs to parents/guardians.

Position Summary

Working from a Family Service Center, the Case Manager is a trained mental health professional who assists with mental health concerns, behavioral concerns, positive behavioral support, and related academic and classroom support for CCA students. The Case Manager consults with teachers, administrators, and caretakers to support the social‑emotional learning and well‑being of all CCA students, individually or in a group. This person is a trusted point of contact between students, families, teachers, and school staff to help students succeed academically and socially.

In order to be successful at CCA, employees must carry out all responsibilities in a manner that demonstrates and models the values of CCA, abides by all standards, and behaves in a professional and engaged manner at all times.

Core Responsibilities
  • Serve as a resource to CCA administrators, counselors, teachers, and other social and health services staff regarding students and to assist in monitoring student achievement and their social‑emotional well‑being.
  • Create and conduct student assessments (intake); implement case management services; collaborate with school staff to develop interventions and individualized service plans.
  • Interpret and provide guidance on student needs including attendance, personal and family matters, academics, policies, laws, and record‑keeping requirements relating to students.
  • Work with students/families to cope with disabilities, trauma, and other behavior concerns.
  • Lead the Student Assistance Program team and assist as needed for students with mental health and substance‑use disorder issues.
  • Monitor student and family progress and status and make visits to residences (including facilities and temporary housing) to assess the family environment as needed.
  • Assist with CCA student attendance and engagement concerns, including participation in school attendance improvement conferences, home visits, and other truancy mitigation and engagement supports.
  • Serve as the school’s assigned Homeless Liaison to support families experiencing homelessness, connecting schools with agencies and facilities such as shelters, social services, court services, and the police department.
  • Assist counselors, administrators, and teachers in creation and implementation of multi‑tiered systems of support and behavior intervention plans.
  • Assist counselors with conflict resolution, behavior management, and targeted small support groups.
  • Provide mentoring support to students in collaboration with other CCA staff.
  • Assist families in accessing internal or external supports; provide information related to the needs of their child and act as a resource to parents/guardians through family support activities and service communication.
  • Assist CCA school counselors in providing emotional and social support to students.
  • Work with truancy team members to:
    • Identify students/families who are truant and provide interventions for chronically absent or school‑avoidant students.
    • Develop community partnerships with county truancy programs.
    • Connect families with local resources and truancy prevention programs.
  • Function as student/family advocates for homelessness, trauma, truancy, and other needs.
  • Support in referrals to health, dental, vision, hearing, mental health, housing, and other appropriate services.
  • Maintain local agency list and contact information.
  • Actively communicate with the Business Office and Federal Programs Manager to discuss funding options.
  • Maintain current knowledge of all legal requirements for providing services, including professional development and review of updates from state and federal Departments of Education; monitor compliance with all laws, regulations, guidelines, school policy, and procedures.
  • Provide staff professional learning and development by conducting training on laws and issues relating to the student population and recommended strategies for supporting these students.
  • Other duties as assigned.

Work Environment

This job operates in a professional office environment. It routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

Physical Demands

This is largely a sedentary role. Requires ability to lift, squat, sit, bend or stand as necessary; ability to travel to other sites/locations as needed.
