Seven Counties Services in Louisville, KY is seeking a Full‑time Case Manager to conduct comprehensive assessments, develop service plans, and arrange needed services based on those assessments.
You will monitor progress, assist with referrals, document case records, and provide psychosocial education. The role may involve crisis assistance and PCC responsibilities depending on skill and autonomy. Vaccinations are required for all employees.

Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or related field as approved by Medicaid, or as required by applicable funding source.
Minimum of zero year’s experience in case management/service coordination with a Bachelor’s degree. Working knowledge of mental and physical characteristics of assigned population group (e. g., growth and development), human behavior, treatment/service options, and related issues. Working knowledge of SCS programs and services, policy, and procedure. Working knowledge of applicable regulations and licensure standards. Good interpersonal skills and written and oral communication skills.
Position requires lifting of 20 pounds maximum, with frequent lifting and/or carrying of items weighing up to 10 pounds, or requires significant periods of walking or standing. Normal tasks may involve exposure to blood, bodily fluids, or tissue. May involve 24‑hour on‑call responsibilities. Reliable transportation for frequent travel.
