Job summary

The Just One Project is seeking a dedicated Case Manager (Senior Services) in Nevada to support older adults with housing, health, food access, and beneficial resources. You will conduct intake, create action plans, and coordinate referrals while maintaining precise HMIS and Salesforce documentation.

This is a full-time, data-driven role requiring strong organizational skills, compassion, and the ability to manage a caseload with competing priorities.

Qualifications

  • Bachelor's degree in Social Work, Human Services, or related field, or equivalent experience
  • Three+ years nonprofit experience
  • Three+ years intensive case management experience
  • Fluency in English and Spanish (written and spoken)
  • Experience providing case management to older adults or vulnerable populations
  • Hands-on HMIS experience for client records and documentation
  • Strong data entry, documentation, and reporting skills
  • Ability to manage an active caseload with deadlines

Responsibilities

  • Conduct intake assessments to identify client needs, goals, and barriers

  • Maintain shared program tracking tools within timelines
  • Review grant/program requirements for client eligibility
  • Prepare client success stories and reports
  • Represent The Just One Project at events including Pop-Up & Give

About the Role

As a Case Manager (Senior Services), you'll provide comprehensive case management services to older adults who may need assistance navigating food access, public benefits, housing, healthcare resources, financial stability, transportation, and other supportive services.

You'll work directly with clients to assess their needs, develop individualized service plans, connect them with appropriate community resources, coordinate referrals, and provide ongoing support as they work toward greater stability, independence, and quality of life.

This is a highly client-facing and data-driven role. You'll balance meaningful direct service with detailed case documentation, including maintaining accurate client records, tracking services and outcomes, and completing required documentation in HMIS and Salesforce within established timelines.

This is a great opportunity for an experienced Case Manager who enjoys working with older adults, understands the unique needs and barriers they may experience, and is equally comfortable building relationships with clients and managing the data and documentation behind their services.

The ideal person for this role is compassionate, patient, highly organized, detail-oriented, comfortable working with data, and able to balance an active caseload with documentation, referrals, follow-up, and changing client needs.

This is a full-time position requiring approximately 40 hours per week, Monday through Friday, 8:00 a.m. to 4:30 p.m. Occasional evening, weekend, event-based, or on-call work may be required depending on operational needs.

What You'll Do
  • Conduct intake assessments to identify client needs, strengths, goals, and barriers to long-term stability
  • Develop individualized action plans with clients focused on housing, health, financial stability, food access, transportation, social support, and other essential needs
  • Provide ongoing case management by monitoring client progress, reassessing needs, evaluating outcomes, and adjusting service plans as needed
  • Conduct regular client assessments and follow-up to ensure clients are receiving appropriate support
  • Connect clients with community resources, including housing programs, healthcare and mental health services, food assistance, transportation, financial assistance, benefits programs, and other supportive services
  • Facilitate referrals and follow up to help clients successfully access available resources
  • Provide crisis support, safety planning, and connections to appropriate emergency resources when necessary
  • Support clients with financial literacy, budgeting, saving, debt management, and other skills that promote greater self-reliance
  • Assess client employment goals, when applicable, and collaborate with workforce partners to connect clients with job training and employment-readiness resources
  • Assist SNAP program participants with applications, required documentation, necessary data, and connections to nutrition education support
  • Maintain the Community Market by restocking shelves, maintaining cleanliness, keeping signage current, and ensuring the space remains welcoming and organized
  • Maintain accurate food inventory records and submit weekly or as-needed food orders to ensure consistent availability
  • Maintain accurate and comprehensive case notes documenting client assessments, services, referrals, progress, outcomes, and relevant correspondence
  • Record required client services, referrals, recommendations, case notes, and program documentation in HMIS or Salesforce within one business day of client interaction
  • Maintain accurate shared spreadsheets and program tracking tools within established timelines
  • Review applicable grant and program requirements to determine client eligibility for available services and assistance
  • Complete required forms, reports, and supporting documentation in accordance with established procedures
  • Participate in internal and external case conferencing to coordinate services and identify solutions for complex client needs
  • Maintain current knowledge of community resources, eligibility requirements, social services, benefits programs, and referral processes relevant to older adults
  • Prepare and submit required client success stories
  • Represent The Just One Project at organization-wide events, including participation in six mandatory Pop-Up & Give events each year
What You Bring
  • Bachelor's degree in Social Work, Human Services, or a related field, or an equivalent combination of education and relevant work experience
  • Three or more years of nonprofit experience
  • Three or more years of intensive case management experience
  • Fluency in both English and Spanish, including verbal and written communication
  • Experience providing case management or supportive services to older adults, seniors, or other vulnerable populations
  • Hands-on experience using HMIS for client records, case notes, services, referrals, and program documentation
  • Strong data entry and documentation skills with the ability to maintain accurate, complete, and timely client records
  • Experience tracking client services, progress, outcomes, and program metrics
  • Experience managing an active caseload while consistently meeting documentation, reporting, and follow-up deadlines
  • Strong assessment, problem-solving, advocacy, and conflict-resolution skills
  • Ability to build trust and rapport with clients while maintaining appropriate professional boundaries
  • An empathetic, nonjudgmental, and trauma-informed approach to client engagement
  • Strong organizational skills and attention to detail with the ability to successfully manage multiple priorities
  • Knowledge of community resources and the ability to connect clients with appropriate services and referrals
  • Strong written and verbal communication skills
  • Ability to maintain confidentiality and appropriately handle sensitive client information
  • Proficiency with Microsoft Office, including Word, Excel, PowerPoint, Outlook, and related workplace applications
  • Ability to work collaboratively with community partners, internal teams, healthcare and social service providers, and other organizations supporting clients
  • Ability to work in an active Community Market environment, including regularly standing, walking, reaching, stocking, and lifting or carrying up to 50 pounds
  • A valid, unrestricted driver's license, reliable personal transportation, and ability to meet organizational insurability requirements
  • Ability to obtain and maintain a valid Nevada Food Handler Card within 30 days of hire
  • Legal authorization to work in the United States

Preferred Qualifications
  • Relevant certifications in case management or conflict resolution
  • Experience specifically providing case management or supportive services to older adults
  • Experience with Salesforce or similar client management systems
  • Familiarity with housing services, wraparound programs, benefits navigation, and self-sufficiency initiatives
  • Proven experience managing a caseload and delivering measurable outcomes in client stability, independence, and self-reliance
  • Familiarity with SNAP program procedures and requirements
  • Knowledge of community resources, benefits, and supportive services available to older adults
