Job summary

Highlands Community Services (HCS) seeks a Case Manager – School-Based to act as the primary liaison between HCS and the school for service provisions and continuity of care. You will participate in intakes to determine consumer needs and collaborate with community partners for integrated support.

Position is full-time with a school-aligned schedule. Bachelor’s in Human Services or related field required; QMHP certification and school experience preferred to enhance collaboration and outcomes.

Qualifications

• Bachelor’s degree in Human Services or related field is required.
• Must be registered as a Qualified Mental Health Professional.
• Experience in a school setting preferred.

Responsibilities

• Serve as primary liaison between Highlands Community Services and the school for service provisions.
• Participate in intakes to determine consumer needs and capabilities.
• Coordinate care with community partners to ensure continuity of services.

Position Specifics

Location: Washington County VA or City of Bristol, VA

Status: Full-time, Non-exempt, Monday-Friday

Compensation: $18.50 - $20.00 – Commensurate with education/experience

Education: Bachelor’s degree in Human Services or related field

Experience: Registered as a Qualified Mental Health Professional and experience in a school setting preferred.
