Job Description

Case Manager RNs are responsible for:

1. Advocating for the patient and family throughout the entire episode of care.
2. Monitoring patient progression and intervening as needed to ensure efficient progress.
3. Coordinating all aspects of discharge planning and establishing a safe discharge plan.

The Case Manager RN utilizes advanced nursing skills and knowledge to coordinate care for a defined patient population, facilitating their journey through the continuum of care.

Qualifications for This Position Include

• Bachelor's of Nursing required
• Three or more years of case management/discharge planning experience
• CCM certification preferred
• Active RN license in NJ/PA
