Job Description

Critical Illness Recovery Hospital

Position: Case Manager

Sign-On Bonus: $10,000

Requires current licensure in a clinical discipline such as Nurse (RN/LPN/LVN), Respiratory Therapist (RT), or Medical Social Worker (license per state guidelines).

Previous experience in critical care is required.

Responsibilities

The Case Manager's responsibilities include utilization review, resource management, discharge planning, treatment and financial management, and medical record documentation. You will report to the Director of Case Management and may provide social work services as per state guidelines.

  • Develop and execute safe, patient-specific, timely discharge plans.
  • Verify utilization criteria.
  • Coordinate with payors to ensure proper reimbursement and promote cost-effective care within the plan of care.
  • Ensure compliance with facility-wide Utilization Management policies.
  • Coordinate with Quality Management to meet licensure and accreditation standards.
  • Manage fiscal responsibilities, maximize reimbursement, and review insurance verification forms to reduce risks.
  • Participate in multidisciplinary team meetings with physicians, nurses, respiratory and rehabilitation therapists.
Qualifications

  • Current licensure in a clinical discipline (Nurse or Respiratory Therapist preferred) or a current social work license per state guidelines.
  • Experience in inpatient hospital settings with critical or acute care patients (e.g., ICU, step-down, med-surg, vents).

Preferred qualifications include:

  • Experience in Care Management and Discharge Planning.
  • Knowledge of insurance and government reimbursement processes.
