Critical Illness Recovery Hospital

Case Manager

$8000 Sign On Bonus

Requires current licensure in a clinical discipline such as Nurse (RN/LPN/LVN), Respiratory Therapist (RT), or Medical Social Worker (license per state guidelines).

Previous experience in critical care required.

Responsibilities

We seek dedicated employees who will champion the Select Medical Way, including prioritizing patient care, improving community quality of life, exploring new ideas, delivering high-quality care, and doing what is right.

The Case Manager handles utilization reviews, resource management, discharge planning, treatment plan management, financial oversight, and medical record documentation. You will report to the Director of Case Management and provide social work services as needed, per state guidelines.

  • Develops and implements safe, patient-specific, and timely discharge plans.
  • Verifies utilization criteria during reviews.
  • Builds relationships and coordinates with payors to ensure proper reimbursement and promote cost-effective, resource-conscious care within the plan of care.
  • Ensures compliance with facility-wide Utilization Management policies and procedures.
  • Coordinates with Quality Management to meet licensure and accreditation standards.
  • Maintains fiscal responsibilities, maximizing reimbursement and insurance benefits, and reviewing insurance verification to minimize risks.
  • Facilitates multidisciplinary team meetings with physicians, nurses, respiratory therapists, and rehabilitation therapists.
Qualifications

  • Experience in inpatient hospital settings with critical or acute care patients (e.g., ICU, step-down, med-surg, vents).

Preferred qualifications include:

  • Knowledge of insurance and government reimbursement processes.
