Job Summary and Qualifications

The Case Manager (CM) ensures high-quality, patient-centered rehabilitative care, managing and coordinating care to achieve optimal outcomes. The CM collaborates with the Rehab Program Director and Facility Case Management Director, coordinating within the rehabilitation team.

Key Responsibilities:

• Provide orientation to patients/families/caregivers about the role, rehab philosophy, stay/discharge criteria, Medicare, insurance, grievance procedures, treatment plans, and rights.
• Coordinate patient/family/caregiver education, promoting participation in planning and goal setting through family conferences and interactions.
• Implement treatment plans aligned with patient needs, strengths, and preferences, facilitating patient involvement.
• Document discharge planning evaluations and psychosocial assessments, communicating social, financial, and discharge needs.
• Promote positive patient interactions to ensure excellent patient service.

Qualifications:

• Registered Nurse (RN)
