Job summary

The Department of Veterans Affairs is seeking a Specialty Case Management RN to lead patient care coordination and discharge planning across inpatient and outpatient settings.

Responsibilities include coordinating care with multiple providers, supporting education and follow-up, and ensuring timely access to services. EDPRP eligibility is noted; strong English proficiency and an active US nursing license are required.

Qualifications

• English proficiency required for written and spoken communication
• Graduate of a nursing program accredited by ACEN or CCNE
• State RN license and eligibility
• Bridge programs may lead to licensure upon completion
• Registration must be active in the U.S. state
• Foreign-educated nurses require degree equivalency validation
• Nurse I-III progression based on experience and education
• Direct patient care experience preferred in outpatient or clinic settings

Responsibilities

• Execute Specialty Case Management with leadership and clinical expertise
• Provide care coordination and discharge planning across inpatient and outpatient settings
• Collaborate with providers and community VA facilities to ensure timely care
• Assist with patient education, triage, and follow-up to meet care goals
• Support cross-unit coverage and maintain clinic nurse competency
