Position Description

The RN Case Manager reports to the Director of Case Management and collaborates with healthcare team members to coordinate quality healthcare delivery at Palmdale Regional Medical Center. The role involves utilization review, discharge planning, and case management for all patients, regardless of their background or payer source, focusing on achieving positive outcomes for patients, providers, the facility, and payers.

Qualifications

• Ability to read, analyze, and interpret professional and technical materials, and communicate effectively with managers, patients, and the public.
• Basic mathematical skills applicable to practical situations.
• Strong reasoning skills, including problem definition, data collection, and conclusion drawing.
• Minimum three years of acute care clinical experience; two years in case management or utilization review preferred.
• Registered Nurse license in California; BLS certification; CPI required within 30 days of hire; Bachelor's degree preferred.
