Position Description

The RN Case Manager, under the guidance of the Director of Case Management, collaborates with healthcare team members to coordinate quality healthcare services at Palmdale Regional Medical Center. The role focuses on achieving positive outcomes for patients, providers, the facility, and payers. The RN Case Manager is responsible for utilization review, discharge planning, and case management across all patient conditions, regardless of background or payer.

Qualifications

Job Duties/Responsibilities Requirements:

• Ability to read, analyze, and interpret professional journals, technical procedures, and regulations; effectively communicate with managers, patients, and the public.
• Ability to apply basic mathematical concepts.
• Strong reasoning skills: define problems, collect data, interpret instructions, and manage variables.
• Minimum three years of acute care clinical experience; two years of clinical case management or utilization review preferred. Demonstrated accountability and teamwork skills.
• Registered Nurse license; Bachelor's degree preferred. Current RN license in California, BLS certification, and CPI certification within 30 days of hire.
