Position Description

The RN Case Manager, reporting to the Director of Case Management, collaborates with healthcare team members to deliver quality healthcare services, focusing on positive outcomes for patients, providers, and the facility. The role encompasses utilization review, discharge planning, and case management for all patients at Palmdale Regional Medical Center, regardless of their background or payer source.

Qualifications

• Ability to read, analyze, and interpret professional and technical documents, and respond effectively to questions from management, patients, and the public.
• Strong mathematical and reasoning skills, with the ability to define problems, collect data, and interpret instructions.
• Minimum of three years of acute care clinical experience; two years in clinical case management or utilization review preferred.
• Registered Nurse with a current California license, BLS certification, and CPI training within 30 days of hire; Bachelor’s degree preferred.
