Case Manager - RN - Inpatient Units - NYP- Queens - Part Time - Weekends

Across NewYork-Presbyterian/Queens, Care Managers are the guardians of continuity of care; they are skilled clinicians leading the full spectrum of patient care, from pre-admission to discharge while ensuring quality outcomes. As a Care Manager at NewYork-Presbyterian- Queens, you'll serve as a highly valued and respected leader from day one, empowered to make a difference for our patients and their families. As a Case Manager, you'll provide intensive care management for adult patients with complex medical needs. You will act as a strong and knowledgeable liaison between the interdisciplinary team—including social workers, physicians, physical therapists, occupational therapists, cardiologists, and intensivists—to support patients and their families. You will manage all aspects of inpatient care, develop individualized care plans, and ensure optimal length of stay. Additionally, you will help prevent hospital readmissions, anticipate outpatient service needs, and coordinate appointments and consultations prior to discharge.

Case Managers actively promote a collaborative relationship with the medical staff to meet case management objectives, such as reducing length of stay (LOS) and providing quality patient care across the continuum.

This role is a weekend day shift position.

Preferred Criteria
  • 2 years of case management or care coordination experience in inpatient, HMO, insurance, or outpatient settings

  • 1 year of leadership experience in a healthcare setting

  • Master's Degree in a healthcare-related field

  • Case Management Certification

Required Criteria
  • Bachelor's degree in Nursing (BSN)

  • Current NYS Registered Nurse (RN) licensure

  • 3 years of clinical experience as a registered nurse in a medical or surgical unit, home care, or post-acute facility
