Case Manager - RN - Inpatient Units - NYP- Queens - Part Time - Weekends

As a care manager at NewYork-Presbyterian- Queens, you'll serve as a highly valued leader from day one, empowered to make a difference for our patients and their families. As a case manager, you'll provide intensive care management for adult patients with complex medical needs. You will act as a knowledgeable liaison between the interdisciplinary team—including social workers, physicians, therapists, and specialists—to support patients and their families. Your responsibilities include developing individualized care plans, optimizing length of stay, preventing readmissions, and coordinating outpatient services and appointments prior to discharge.

This role promotes collaboration with medical staff to achieve case management objectives, reduce length of stay, and ensure quality care across the continuum.

This position is a weekend day shift role.

Preferred Criteria

  • 2 years of case management or care coordination experience in inpatient, HMO, insurance, or outpatient settings

  • 1 year of leadership experience in a healthcare setting

  • Master's Degree in a healthcare-related field

  • Case Management Certification

Required Criteria

  • Bachelor's degree in Nursing (BSN)

  • Current NYS Registered Nurse (RN) licensure

  • 3 years of clinical experience as a registered nurse in a medical or surgical unit, home care, or post-acute facility

$114,500 - $169,000 annually
