We are seeking an experienced Hospice Nurse for this position.
Shifts/Hours: (5) 8-hour day shifts
Job Summary
Performs professional nursing care for patients in a primary care setting (home or alternate site) according to nursing practices, agency policies, and regulations. Provides nursing services to patients and families within the scope of RN practice as defined by the North Carolina Board of Nursing.
Major Job Responsibilities
- Plan, provide, supervise, and document professional nursing care utilizing the nursing process, following the NC nurse practice act, physician orders, and policies. Individualize care based on assessment of patient needs and responses.
- Use assessment skills to prioritize and set outcomes; collaborate with healthcare team to evaluate and optimize patient care.
- Assess educational needs of patients/families regarding health alterations, disease, and care plans.
- Develop and implement discharge plans from the start of services.
- Provide educational resources and interventions; evaluate teaching outcomes.
- Model professional nursing conduct, appearance, communication, ethics, critical thinking, and problem-solving.
- Participate in interdisciplinary team meetings and case conferences.
- Stay updated on Medicare/Medicaid/JCAHO standards through training.
- Assess symptom severity, communicate findings, and intervene as needed.
- Document care accurately and timely, in accordance with policies and regulations.
- Perform medication reconciliation and education to prevent adverse events.
- Observe and report medication reactions and incidents.
- Communicate with team members, caregivers, and physicians regularly.
- Follow infection control protocols and use PPE appropriately.
- Delegate and supervise care by NCA's/LPN's.
- Develop and assign care plans for aides based on patient needs and aide skills.
- Identify learning needs and participate in staff development.
- Maintain professional competencies and seek educational opportunities.
- Implement agency policies and collaborate with billing staff for accuracy.
- Maintain confidentiality of patient and employee information.
- Promote a positive workplace environment.
- Maintain a professional appearance.
- Participate in Quality Improvement initiatives.
- Support comprehensive team-based services.
- Perform additional duties as assigned.
Work Environment and Requirements
Primarily provide patient care in the patient's residence or skilled nursing facility (>50%). Travel to and from patient residences is required.
Preferred Qualifications
Education and Formal Training
- Associate Degree or Diploma in Nursing required.
- Encouraged to enroll in a DUHS-approved BSN program after two years of service; must complete within five years.
Licensures, Certifications
- Current North Carolina RN Licensure or compact state license required.
- Current CPR certification (American Heart Association).
Professional Experience
- Minimum of two years in med-surgical, critical care, or home health/hospice/infusion.
- Valid driver’s license and auto insurance.
- Experience in Home Health, Hospice, or Infusion preferred.
- OASIS and PDGM experience preferred.
Knowledge, Skills & Abilities
- Knowledge of Medicare/Medicaid regulations, JCAHO, ICD-9 coding.
- Understanding of RN scope of practice and nursing process.
- Ability to assess and prioritize patient needs.
- Effective communication and collaboration skills.
- Teaching ability for patients and families.
- Proficiency with documentation and technology.
- Wound care, infusion, and advanced nursing skills knowledge.
- Critical thinking and organizational skills.
- Knowledge of infection control and safety procedures.
- Strong computer skills (Outlook, Word, Excel).
- Ability to work independently and withstand driving and physical demands.
Minimum Qualifications
Graduation from an accredited BSN program. Registered nurses hired without a BSN between July 1, 2014, and April 11, 2021, are encouraged to enroll in a BSN program within two years of start date and complete it within five years. Nurses hired before July 1, 2014, are not required to enroll.
At least 12 months of relevant clinical experience required; exceptional performers with 6 months may be considered. Opportunities for promotion to CN II after 12 months, contingent on program completion and performance.
Licenses and Certifications
Valid RN license in North Carolina or a compact state, BLS certification required.
