Job summary

WelbeHealth PACE is seeking a Case Manager, RN to guide participant care within a value-based model. You will conduct nursing assessments, develop and follow personalized care plans, coordinate with IDT, and support transitions of care, with visits to participants in their homes.

The role offers a Monday–Friday schedule with no weekends or on-call, opportunities for career growth, and a comprehensive benefits package including medical/dental/vision and a 401(k) match.

Qualifications

• RN licensure in New Jersey required
• BLS/First Aid certification required
• CPR and First Aid certifications to be obtained within 180 days of hire
• Two years of nursing experience with frail or elderly population preferred
• BSN preferred for enhanced care planning and leadership

Responsibilities

• Conduct nursing assessments to determine participant needs, preferences, and goals with IDT input
• Deliver and document nursing interventions per care plans
• Coordinate with WelbeHealth PACE Physician and IDT to revise care plans as needed
• Educate participants and families on disease processes, self-care, and prevention strategies
• Visit participants in their living environments for routine follow-ups and nursing care as needed

Job description

The Case Manager, RN directly guides participant care as a crucial voice on the interdisciplinary team (IDT). Reporting to clinical leadership, the Case Manager, RN delivers and oversees all aspects of care, including conducting required PACE assessments, developing and following personalized participant care plans, and supporting transitions of care coordination.

On the day-to-day, you will:

• Conduct clinic nursing assessments to determine participant needs, preferences, and goals, and actively participate in IDT meetings to develop participant care plans
• Deliver and document nursing interventions as agreed upon in participants’ care plans, including but not limited to maintaining a safe environment, promptly responding to physician orders, and correctly administering medications
• Collaborate with the WelbeHealth PACE Physician and other members of the IDT in coordinating participant care needs, as well as communicate with participants and their families to revise care plans
• Instruct participants, their families, and caregivers regarding the disease process, self-care techniques, and prevention strategies
• Ability to visit participants in their living environment for routine follow ups and perform necessary nursing care as needed

Job requirements include:

• Graduate of an accredited school of nursing; Bachelor of Science (BSN) preferred
• Unencumbered New Jersey RN License
• Current Basic Life Support (BLS)/First Aide Certification required
• Two (2) years of nursing experience in a clinical setting with a frail or elderly population preferred
• Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
