WelbeHealth is seeking a Case Manager, RN in New Jersey to lead participant care within our PACE program. You will conduct nursing assessments, participate in IDT meetings, and develop personalized care plans to support home-based care for seniors.

The Case Manager, RN directly guides participant care as a crucial voice on the interdisciplinary team (IDT). Reporting to clinical leadership, the Case Manager, RN delivers and oversees all aspects of care, including conducting required PACE assessments, developing and following personalized participant care plans, and supporting transitions of care coordination.
Compensation consists of base salary plus bonus. WelbeHealth offers a competitive total rewards package that includes a 401(k) match, comprehensive healthcare coverage, and a broad range of additional benefits. Actual compensation will be determined based on experience and relevant qualifications.
