A leading healthcare organization is seeking a Case Manager to oversee clinical care and financial planning for patients. The role requires a compassionate approach towards patient and family support, as well as strong nursing skills. Qualified candidates will have a Bachelor's degree in Nursing and three or more years of clinical experience. This position is critical in ensuring quality care outcomes within a dedicated healthcare team.
Under the general supervision of the Manager, Case Management, the role involves coordinating the clinical and financial plan for patient assignments, performing utilization management, resource management, discharge planning, care facilitation, and referrals to other levels of care. The incumbent works with physicians and the interdisciplinary team to achieve quality and financial outcomes.
The candidate must demonstrate the knowledge and skills necessary to provide care appropriate to the age, culture, ethnicity, and specific diagnostic needs of patients served. They should be able to assess and interpret patient data to identify needs and provide appropriate care for specific patient groups (pediatric, adolescent, geriatric, culture, and diagnostic specific).
It is essential to consistently demonstrate compassion and courtesy towards patients and extend support to their families and visitors.
