Job summary

Comprehensive Community Action in Cranston, RI, is seeking a dedicated case manager to provide services under the RI Works program. You will assess clients, develop service plans, and help resolve barriers to progress.

You will conduct home visits, assist with emergency funding and benefits applications, maintain accurate records, and collaborate with CCAP programs to coordinate resources. A valid driver's license and willingness to travel are required.

Qualifications

  • Bachelor's degree required; preferred.
  • Minimum of 2 years related experience.
  • Must have demonstrated case management skills and experience.
  • Experience with emergency intervention methods, family dynamics, health, mental health, and substance abuse systems.
  • Knowledge of community resources.
  • Valid driver’s license and proof of liability insurance.
  • Reliable transportation.
  • Provide/maintain all required immunizations and/or vaccinations.
  • Complete all required background checks.
  • Bilingual in Spanish and/or Creole; preferred.

Responsibilities

  • Conduct comprehensive assessments (including risk factors) on assigned clients.
  • Develop a service plan with clients as per program provisions.
  • Provide community and/or home visitation to clients on caseload.
  • Provide instruction in adult daily living skills related to barriers for goals.
  • Assist with emergency funding, housing, utility assistance, medical and state benefits applications.
  • Maintain client contact as deemed by program/DHS and record keeping per DHS standards.
  • Utilize database systems to maintain client information per agency requirements.
  • Complete paperwork for data generation in a timely fashion.
  • Collaborate with CCAP programs and administration to coordinate efforts and resources.
  • Attend program-related meetings and CCAP PR activities.
  • Attend and participate in training and development sessions.

Job Summary:

To provide case management and care coordination services to individuals and families within the RI Works program, ensuring their compliance for specific component areas and assisting with resolving barriers for progress.

Requirements
REQUIRED QUALIFICATIONS:

Bachelor's degree; preferred

Minimum of 2 years related experience

Must have demonstrated case management skills and experience; emergency intervention methods, family dynamics, health, mental health, and substance abuse systems; and knowledge of community resources

Valid Driver’s License and proof of liability insurance

Programmatic:
  • To conduct comprehensive assessments (including assessing risk such as but not limited to basic human needs, vocational/employment factors, mental health/substance use and other related risk) on assigned clients.
  • To develop a service plan with clients in accordance with provisions stated within the specific case management program.
  • To provide community and/or home visitation to clients on caseload.
  • To provide instruction in adult daily living skills as needed and as related to barriers for academic/vocational/employment goals.
  • To assist with completion of emergency funding, housing, utility assistance, medical and state benefits and other appropriate applications for clients as needed.
  • To maintain required client, contact as deemed by program/DHS.
  • Maintain current and accurate record keeping for client files in accordance with standards of DHS
  • To utilize computer database systems to maintain appropriate information on clients, per program and agency requirements.
  • To complete all paperwork associated with maintaining current client statistics.
  • Ensure agency standards of care are met and in compliance with funders and others as required.
  • To complete all paperwork required for data generation in a timely fashion
  • Collaborate with other CCAP programs and administration to ensure coordination of efforts and resources and the smooth implementation of services.
  • Attend program-related meetings as appropriate.
  • Attend and participate in all CCAP public relations activities as appropriate.
  • Attend training and development sessions
WORK SCHEDULE DEMANDS:
  • Full time; 35 hours/week.
  • Due to the nature of the job, this individual must be flexible with their schedule to accommodate the client population.

While performing the duties of this job, the employee is regularly required to sit and to talk to hear. The employee must occasionally lift and / or move up to 20 lbs. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. The environment is a clinical business office environment. Potential exposure to blood borne pathogens, respiratory diseases and other infectious diseases, and other biohazardous material requiring universal standard precautions. This position requires the individual to have their own transportation, a current unrestricted license to drive, documented auto insurance and the individual must be willing to work in schools, client’s homes, and the community.
