Qualifications

• Two+ years in case management, workforce services, social services, or related field.
• Experience working with individuals facing employment barriers.
• Strong documentation habits and comfort with case management systems.
• Ability to build trust quickly with participants and hold them accountable.

Responsibilities

• Carry a caseload of participants from intake through 90-day retention follow-up
• Conduct needs assessments and build individual service plans
• Coordinate wrap-around services and connect participants to community resources
• Maintain regular contact with placed workers and their employers to identify problems early
• Document all participant contact, services delivered, and outcomes
• Escalate systemic barriers to program leadership
• Support hiring fairs, orientation sessions, and community events
