Job summary

South Middlesex Opportunity Council (SMOC) in Framingham, MA seeks a Case Manager for Residential Recovery to provide individual and group counseling, develop treatment plans, and coordinate services for clients in the program. This full-time role supports housing and employment stability and requires collaboration with the Clinical Supervisor.

The position emphasizes delivering client-centered care, maintaining thorough records, and advocating for clients within the Housing Court and community

Qualifications

  • Working with female clients; designation as female only due to privacy and safety requirements.
  • Minimum of one year experience in Human Services or related field.
  • Certification or license in substance abuse counseling (CADAC or LADAC) or enrollment in a certificate program.
  • If applicable, a minimum of 3 years continuous sobriety.
  • Minimum one year assessment, advocacy and case management skills.
  • Sensitivity to low-income people of diverse backgrounds; excellent listening skills.
  • Ability to work independently and collaboratively.
  • Strong written and verbal communication skills.
  • Must have valid driver’s license, reliable transportation and meet insurance standards.

Responsibilities

  • Provide individual and group counseling to assigned clients; collaborate with Clinical Supervisor as needed.
  • Establish a treatment plan for each individual.
  • Provide information and referrals to community resources (Fuel Assistance, Food Stamps, food pantries, etc.).
  • Coordinate all services required by the treatment plan.
  • Ensure clients comply with program guidelines and complete duties.
  • Maintain complete and accurate client files and records; complete paperwork timely.
  • Collaborate with SMOC staff on housing and employment search and stability.
  • Maintain weekly contact and reassess adherence to the treatment plan.
  • Support clients in economic development planning.
  • Communicate monthly as needed and mediate conflicts; advocate in Housing Court.
  • Maintain up-to-date case notes and data entry in required databases.
  • Participate in Case Management team meetings and trainings.
  • Conduct groups and workshops as needed.
  • Arrange appointments and transportation for clients.
  • Engage clients by addressing needs within or outside scope of work.

Tuesday through Saturday: 8:00AM - 4:00PM. Includes a 1-hour unpaid lunch break.
