Job summary

Volunteer Behavioral Health seeks a qualified professional to supervise residential services, assess and plan care for consumers within 14 days of admission, and coordinate discharge planning. You will manage a caseload, conduct regular visits, and connect clients to community resources.

Qualifications

• Bachelor's degree in health-related field and at least two years of paid experience in behavioral health preferred.
• Must have a valid driver's license with a clean driving record.

Responsibilities

• Screen consumers referred from RMHI for Supervised Residential services.
• Conduct and document assessments within 14 days of admission.
• Develop Service Plans within 14 days of admission.
• Educate and connect consumers to community resources.
• Assist consumers in obtaining benefits and accessing services.
• Prepare Crisis Action Plans and related documents within 14 days.
• Maintain a caseload of at least 8 consumers.
• Perform at least four case management visits per consumer per month.
• Coordinate and document weekly community outings for residents.
• Provide daily living/assistance training at least 3 times weekly.
• Provide education/training sessions at least twice weekly.
• Submit Continued Stay Reviews by the suspense date.
• Maintain required documentation and training records.
• Attend mandatory trainings including annual and risk-area compliance.
