Job summary

CareStar, Inc. is seeking a Case Manager for Clinton, Greene, Warren, and Clermont Counties in Ohio. The role combines remote work with field visits to assess needs, develop care plans, and connect clients with essential services.

You will collaborate with a mission-driven team to improve lives and support community-based care coordination. The position requires a Bachelor’s degree in a related field and active licensure (LSW/LISW/LPN/RN), plus at least one year in home and community-based

Qualifications

• Bachelor’s degree in Social Work, Counseling, Psychology, Nursing, or related field
• Licensed Social Worker (LSW/LISW/LPN/RN) is required
• Minimum 1 year of home and community-based services experience
• Experience with Medicaid, behavioral health systems, and local resources

Responsibilities

• Conduct face-to-face eligibility evaluations and assessments
• Collaborate to develop individualized care plans and safety plans
• Coordinate services across behavioral health, medical, and community systems
• Assist with transitions and care coordination during provider changes
• Ensure regulatory reporting and incident documentation as needed
• Maintain confidentiality and adhere to policies and procedures
