Position Summary:
This is a field-based position with 50-75% travel to meet members face-to-face in South Cook County and surrounding areas. The work schedule is Monday to Friday, 8am-5pm CST. The role involves managing a full caseload of waiver members, requiring in-person quarterly visits and meeting contractual requirements.
Responsibilities include facilitating healthcare outcomes, providing care coordination, support, and education, and conducting comprehensive evaluations of members' needs and eligibility. The role also involves identifying high-risk factors, coordinating care plans, and monitoring progress. Additionally, the nurse will consult with other healthcare professionals, address quality of care issues, and utilize motivational interviewing to promote lifestyle changes.
Qualifications include at least 2 years of clinical experience, an active Illinois RN license, reliable transportation, and willingness to travel. Preferred qualifications include case management and discharge planning experience, managed care experience, and proficiency with Microsoft Office. An associate's degree is required; a BSN is preferred.
The position offers a full-time schedule, with an anticipated weekly hours of 40, and a pay range of $66,575 to $142,576, depending on experience and other factors. Benefits include medical plans, 401(k), stock purchase plans, wellness programs, paid time off, flexible schedules, and more.
