Case Manager Registered Nurse - Field (South Cook County, IL and surrounding)

Join us as a Case Manager Registered Nurse - Field (South Cook County, IL and surrounding) at The Muse.

Position Summary: This is a field-based role with 50-75% travel to meet members face-to-face in South Cook County and surrounding areas. Work hours are Monday to Friday, 8am-5pm CST. The position manages a full caseload of waiver members and requires quarterly in-person visits to members. It is essential for meeting contractual requirements.

Responsibilities include:

• Conduct comprehensive evaluations of members' needs and eligibility using care management tools and data review.
• Identify high-risk factors and coordinate care plans, referring to clinical case management or crisis intervention as needed.
• Implement and monitor care plan activities, ensuring progress and quality of care.
• Collaborate with healthcare teams, present cases at conferences, and escalate quality issues.
• Utilize negotiation and motivational interviewing skills to enhance member engagement and promote healthy lifestyle changes.
• Provide coaching and support to empower members in healthcare decision-making.

Qualifications:

• Minimum 2 years of clinical experience.
• Active Illinois RN license in good standing.
• Reliable transportation and willingness to travel 50-75% of the time; valid Illinois driver’s license required. Mileage reimbursed.
• Preferred: Experience in case management, discharge planning, and managed care. Associate degree minimum; BSN preferred.

Additional Details:

• Full-time, 40 hours/week.
• Salary range: $66,575 - $142,576, based on experience and qualifications.
