This position is with CVS Health's Aetna team, focusing on integrated care management in the Southern and Lower Central Region of New Jersey, preferably Ocean, Monmouth, or Atlantic County. The role involves face-to-face assessments, care planning, and coordination for members with various health needs.

Key Responsibilities:

• Conduct telephonic and face-to-face assessments in members' homes, hospitals, behavioral, and long-term care facilities.
• Develop and monitor member-centric care plans in collaboration with members and care teams.
• Coordinate services and ensure timely delivery of care, addressing social needs as necessary.
• Maintain accurate documentation in electronic health records.
• Travel 50-75% to meet members face-to-face, using reliable transportation.

Required Qualifications:

• RN with current unrestricted New Jersey license.
• Reside in Southern and Lower Central Region of New Jersey, preferably Ocean, Monmouth, or Atlantic County.
• 3+ years' experience with pediatric, special needs, or adult populations.
• 2+ years managing mental health/substance use disorders.
• Ability to travel as needed.
• Proficiency in computer systems and MS Office.

Preferred Qualifications:

• Certified Case Manager Certification.
• Experience in an integrated care model.
• Bilingual skills.
• Strong communication and problem-solving skills.
• Knowledge of medical terminology and digital literacy.

Education: Bachelor's in Science & Nursing required.

Work Hours: 40 hours/week, full-time.

Pay Range: $72,627 - $155,538 annually, dependent on experience and location. Eligible for bonuses and incentives.
