Position Summary

The Care Manager will develop, implement, support, and promote health service strategies, policies, and programs that ensure the delivery of quality healthcare to our members. Responsibilities include utilization management, quality management, network management, clinical coverage, and policies. The role requires advanced clinical judgment and critical thinking skills to facilitate appropriate physical, behavioral health, and psychosocial wrap-around services. The care manager will be responsible for care planning, direct provider collaboration, and resource utilization in a cost-effective manner. Strong assessment, writing, and communication skills are essential.

Fundamental Components / Job Description

The Case Manager is responsible for conducting face-to-face visits in the members' homes, using comprehensive assessment tools for members enrolled in Managed Long-Term Services and Supports (MLTSS) and/or Dual Special Needs Program (D-SNP/FIDE). They may also assess non-MLTSS members to evaluate medical needs and facilitate overall wellness, addressing requests such as adult or pediatric medical daycare, personal care, nursing facility requests, and program enrollment. Completion of the NJ Choice Certification is required to maintain employment. The case manager will coordinate and collaborate with members, authorized representatives, PCPs, and care teams, schedule and attend interdisciplinary meetings, and advocate for proper discharge and services. They will develop care plans, authorize services within benefit guidelines, document accurately in electronic health records, and mentor new hires once proficient. Travel of 50-75% within Atlantic and Cape May counties and surrounding areas is expected.

Required Qualifications
  • Active and unrestricted RN license in NJ
  • Must reside in Atlantic or Cape May County, NJ
  • At least 3 years of clinical practice experience (hospital, home health, ambulatory care, etc.)
  • Reliable transportation and willingness to travel 50-75%; mileage reimbursed
  • Successful completion of NJ Choice certification (score 80% or higher) is mandatory
Preferred Qualifications
  • Case management or discharge planning experience
  • Experience with LTSS and managed care
  • Crisis intervention skills
  • Proficiency with computers and Microsoft Office (1+ year)
Education
  • Associates Degree in Nursing

Weekly hours: 40

Time type: Full-time

Pay Range: $72,627 - $155,538, depending on experience, education, and location. The role is eligible for bonuses, commissions, or incentives.
