    Position Summary

    Job description:

    Develop, implement, support, and promote health service strategies, tactics, policies, and programs that drive the delivery of quality healthcare to our members. These strategies include utilization management, quality management, network management, clinical coverage, and policies. The position requires advanced clinical judgment and critical thinking skills to facilitate appropriate physical, behavioral health, and psychosocial wrap-around services. The care manager will be responsible for care planning, direct provider collaboration, and effective utilization of resources in a cost-effective manner. Strong assessment, writing, and communication skills are essential.

    Fundamental Components/Job Description:

    The Case Manager is responsible for conducting face-to-face visits in members' homes using comprehensive assessment tools for members enrolled in Managed Long-Term Services and Supports (MLTSS) and/or Dual Special Needs Program (D-SNP/FIDE). They may also assess non-MLTSS members to evaluate medical needs and facilitate overall wellness, addressing services such as adult and pediatric medical daycare, personal care assistance, nursing facility requests, personal preference programs, and MLTSS enrollment. Successful completion of the NJ Choice Certification is required for employment. The case manager will coordinate and collaborate with members, providers, and care teams, develop care plans, and authorize services within the MLTSS/FIDE benefits. Accurate and timely documentation in the electronic health record is essential. The role involves critical thinking, problem-solving, and mentoring new hires after proficiency is attained. Travel of 50-75% within Atlantic/Cape May Counties and surrounding areas is expected.

    Required Qualifications

    • Active and Unrestricted RN license in NJ
    • Must reside in Atlantic or Cape May County, NJ
    • 3+ years of clinical practice experience in settings such as hospitals, home health, or ambulatory care
    • Reliable transportation and willingness to travel up to 75%; mileage reimbursed
    • Successful completion (score of 80% or higher) of NJ Choice certification is mandatory

    Preferred Qualifications

    • Case management and/or discharge planning experience
    • LTSS experience
    • Managed Care experience
    • Crisis intervention skills
    • Proficiency with computers and Microsoft Office for 1+ years

    The typical pay range for this role is $72,627.00 - $155,538.00. The actual offer depends on experience, education, location, and other factors. The position may include bonuses, commissions, or incentives.
