Job summary

Novant Health Presbyterian Medical Center in Charlotte, NC is seeking an RN Case Manager to lead complex discharge planning for ICU patients. You will work with the care team to ensure safe, proactive discharge plans and coordinate with MSW case managers for psychosocially complex cases.

Ideal candidates hold an associates degree in nursing with RN licensure in NC and 1+ year of acute care experience; a BSN is preferred.

Qualifications

• Associates Degree in Nursing is required; Bachelor's Degree in Nursing preferred.
• Minimum 1 year of acute care RN experience strongly preferred; hospital or rehab case management helps.
• Current RN licensure in the state of employment; holiday coverage requirements.

Responsibilities

• Complex discharge planning.
• Collaborate with physicians and the healthcare team to assess, plan, implement, and evaluate discharge plans.

• Maintain responsibility and accountability for care management.

What We Offer

As an RN Case Manager at Novant Health Presbyterian Medical Center, you will serve as a leader of the complex discharge planning process and a valued member of the healthcare team. This opening will support the needs of our ICU patients by providing resources and working to safely discharge.

Presbyterian Medical Center is our 661 bed level 2 trauma center centrally located just outside of uptown Charlotte. This role is ideal for an experienced acute care RN looking to work in a patient facing but non-bedside role within a high-performing, collaborative team.

What You’ll Do

• Complex discharge planning
• Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive/complex/safe discharge plans
• Collaborate with MSW case managers for guidance with psychosocially complex cases
• Facilitate coordination of care
• Accept responsibility, authority, and accountability for management of care
• Serve as a resource for discharge planning and care coordination for medically complex cases
• Maintain current knowledge of available community resources/post acute care options

What We’re Looking For

• Education: Associates Degree in Nursing required. Bachelors Degree in Nursing preferred.
• Experience: Minimum 1 year of acute care RN experience strongly preferred. Experience in hospital or rehab case management preferred. Broad acute care bedside experience is a plus.
• Licensure/Certification: Current RN licensure in state of employment, required.
• Additional Skills/Requirements (required): Holiday coverage
